New Patient Forms
In order to make your visit as smooth as possible, we've provided the new patient forms we'll need at your first visit. The forms linked above allow you to fill them out in the comfort of your own home and bring them with you to your visit. Your privacy is important to us. Look over our Patient HIPPA Consent and Consent to Proceed forms. What to Expect We recommend that you come in 15 minutes early to fill out the new patient forms if you haven't filled them out ahead of time. If you have dental insurance please make sure you bring your insurance card. In order to streamline the process, please call our friendly front desk staff and they will enter your insurance information into our system to verify your benefit coverage. During your first visit you will receive a comprehensive exam, x-rays and cleaning. We will check for cavities, gum disease and perform an oral cancer screening. We will also review and explain your customized treatment plan. |

What an experience! One day I'm in for the 6 month checkup, next day I'm in for a cavity and a lingual frenulum (tongue tied) procedure. Between the gas, movie overhead, mouth forced open, I was still able to carry on with their conversation. Amazing what they understand when trying to communicate even if my answers were minutes later. No one likes the dentist! My experience was entertaining and the next thing I know...they were kicking me out! And for a bonus!.....they sent me home with a letter with permission to whine and complain for 1 week post dental treatment. Great service, great staff with all the entertainment included!
